Job summary

Sweetwater Care is seeking a licensed Physical Therapist to perform treatments and exercises to help patients recover from illnesses and injuries. You will develop personalized treatment plans, prescribe exercises, and educate patients to improve mobility.

This role involves supervising physical therapist assistants, coordinating with the rehabilitation team, maintaining accurate records, and ensuring compliance with regulatory requirements.

Qualifications

• Licensed physical therapist with active state license.
• Experience in physical therapy assessment and treatment planning.
• Ability to supervise assistants and collaborate with the care team.
• Strong documentation and regulatory compliance.

Responsibilities

• Consult with patients to learn about their physical condition and symptoms.
• Diagnose movement dysfunction by observation.
• Develop individualized treatment plan.
• Coach patient and teach therapeutic exercises to ease pain and improve mobility.
• Maintain patient records and document goals in the clinical record.
• Assess progress and adjust treatments accordingly.
• Provide discharge summaries per regulatory requirements.
• Document all regulatory and clinical information in electronic records.
• Supervise and support physical therapist assistants and rehabilitation aides.
• Adhere to company policy and code of conduct.
• Active State Licensure.
