Job summary

Aura Care Group in Minnesota is seeking a dedicated Physical Therapist to evaluate, plan, and implement physical therapy services to restore function, mobility, and independence for residents. The PT will collaborate with an interdisciplinary team, develop treatment plans, provide therapies, educate families, document progress, and supervise PTAs as appropriate.

Qualifications

• Minnesota license required.
• Graduate of an accredited physical therapy program.
• Strong communication and teamwork skills.

Responsibilities

• Evaluate patients' physical abilities, needs, and goals through assessments.
• Develop individualized treatment plans based on physician orders and best practices.
• Provide direct physical therapy treatments including therapeutic exercise and gait training.
• Educate patients, families, and caregivers on safe mobility and home programs.
• Monitor progress and update treatment plans to maximize outcomes.
• Collaborate with physicians, nursing, and other therapy professionals for coordinated care.
• Supervise and guide Physical Therapy Assistants as appropriate.

Education
