Job summary

Aura Care Group in Minnesota is seeking a dedicated Physical Therapist (PT) to evaluate, plan, and implement therapy services for residents. You will collaborate with an interdisciplinary team to restore function, improve mobility, relieve pain, and promote independence.

You will develop individualized treatment plans, provide direct treatments, supervise PTAs as appropriate, educate patients and families, and ensure compliance with state and federal standards.

Qualifications

• Current and active Physical Therapist license in Minnesota.
• Graduate of an accredited physical therapy program.
• Strong clinical skills with rehabilitation techniques, equipment, and safety standards.
• Excellent communication, documentation, and interpersonal skills.
• Ability to work effectively in a team environment.

Responsibilities

• Evaluate patients’ physical abilities, needs, and goals through assessments and screenings
• Develop individualized treatment plans in accordance with physician orders and clinical best practices
• Provide direct physical therapy treatments including therapeutic exercise, gait training, balance, and mobility interventions
• Train patients, families, and caregivers on safe mobility, home exercise programs, and use of assistive devices
• Monitor and document patient progress, updating treatment plans as needed to maximize outcomes
• Collaborate with physicians, nursing, and other therapy professionals to ensure coordinated care
• Maintain compliance with federal, state, and professional practice standards
• Supervise and guide Physical Therapy Assistants (PTAs) as appropriate

Position Summary

The Physical Therapist (PT) is responsible for evaluating, planning, and implementing physical therapy services to restore function, improve mobility, relieve pain, and prevent or limit permanent disability. The PT collaborates with the interdisciplinary team to ensure individualized, resident-centered care that promotes health and independence.

Key Responsibilities
