Job summary

Aura Care Group is seeking a Physical Therapist (PT) to evaluate, plan, and implement therapy services aimed at restoring function and improving mobility for residents. You will collaborate with an interdisciplinary team to ensure patient-centered care and safety in a skilled nursing environment.

The role emphasizes evaluation, plan development, direct therapy, caregiver training, and ongoing progress documentation.

Qualifications

• Active Physical Therapist license in Minnesota.
• Graduate of an accredited physical therapy program.
• Strong clinical skills with rehabilitation techniques and safety standards.
• Excellent communication, documentation, and interpersonal skills.
• Ability to work effectively in a team environment.
• Experience in skilled nursing or long-term care setting preferred.

Responsibilities

• Evaluate patients’ physical abilities, needs, and goals through assessments and screenings.
• Develop individualized treatment plans in accordance with physician orders and clinical best practices.
• Provide direct physical therapy treatments including therapeutic exercise, gait training, balance, and mobility interventions.
• Train patients, families, and caregivers on safe mobility, home exercise programs, and use of assistive devices.
• Monitor and document patient progress, updating treatment plans as needed to maximize outcomes.
• Collaborate with physicians, nursing, and other therapy professionals to ensure coordinated care.
• Maintain compliance with federal, state, and professional practice standards.
• Supervise and guide Physical Therapy Assistants (PTAs) as appropriate.
