Job summary

Baptist Health Rehab Services is seeking a full-time Physical Therapist for day shifts in Lexington, KY. The role involves evaluating and treating patients, developing individualized treatment plans, and educating patients and families on goals and plan of care.

The PT will document according to state and national regulations, supervise students and rehab staff, and contribute to departmental operations while addressing patients with varied needs.

Qualifications

• Bachelor's Degree in Physical Therapy.
• Current license by the Ky Board of Physical Therapy.
• BLS within 30 days from date of hire.

Responsibilities

• Completes appropriate chart / medical record review.
• Follows plan of treatment appropriately, notifies referring provider of change in status and need for modifications to treatment plan or goals.
• Plans treatment appropriately, including input from patient.
• Receives and implements therapy orders.
• Completes evaluation form, treatment notes, progress reports, and D/C summaries in a timely manner, and appropriately submits to referring provider.
• Supervises/delegates/directs activities of students and Rehab Services Technicians.
