Job summary

Baptist Health Rehab Services is seeking a Physical Therapist for a full-time, day shift position in Lexington, KY. The therapist will evaluate and treat patients, develop treatment plans, set goals, and document according to regulations and policies.

Qualifications

• Bachelor's Degree in Physical Therapy required.
• Current license by the Ky Board of Physical Therapy required.
• BLS within 30 days from date of hire.

Responsibilities

• Completes appropriate chart / medical record review.
• Follows plan of treatment, notifies provider of change in status and need for modifications to treatment plan or goals.
• Plans treatment with input from patient.
• Receives and implements therapy orders.
• Completes evaluation forms, treatment notes, progress reports, and discharge summaries in a timely manner.
• Supervises/delegates/directs activities of students and Rehab Services Technicians.
