Job summary

Anberry Post Acute in Merced, CA seeks a Physical Therapist to provide and direct all aspects of patient care, including evaluation, treatment planning, goal setting, family education and documentation in a 24‑hour skilled care environment. You will develop treatment plans, educate staff and third‑party payers on outcomes, ensure safe practice, and supervise assistants per state and facility guidelines.

Qualifications

• Graduate of an approved curriculum in Physical Therapy.
• Current, active license as Physical Therapist.
• CPR Certification, as required for license renewal.
• Experience in long term care preferred.
• Must be able to read, analyze, and interpret common scientific information and communicate in English.

Responsibilities

• Evaluations initiated per MD orders and facility/payer guidelines.
• Develops treatment plans and provides treatment for best functional outcomes.
• Educates staff and third party payers on benefits and outcomes of intervention.
• Completes documentation including evaluations, daily progress notes, and monthly reports.
• Maintains patient safety and works with rehab team to implement programs.
