Job summary

TreVida Healthcare in Steubenville, OH seeks a compassionate Physical Therapist to evaluate residents, develop treatment plans, and provide therapy to improve mobility and independence. You will work with an interdisciplinary team to maximize outcomes and deliver safe rehabilitation services.

Responsibilities include gait training, documentation, and educating residents and families. Ohio licensure and PT program graduation are required; clinical experience in skilled nursing is preferred.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in the State of Ohio.
• Strong communication and interpersonal skills.
• Ability to collaborate with residents, families, physicians, and interdisciplinary staff.
• Knowledge of rehabilitation techniques and documentation standards.

Responsibilities

• Evaluates residents and determines PT needs and goals.
• Develops individualized treatment plans based on physician orders.
• Provides skilled PT services to improve strength, mobility, balance, and independence.
• Assists with gait training, transfers, positioning, and mobility devices.
• Documents evaluations, treatment plans, progress notes, and discharge summaries.
• Collaborates with physicians, nursing staff, and therapy team on care plans.
• Monitors response to treatment and adjusts programs as needed.
• Educates residents, families, and staff on therapy programs and safety.
• Participates in care plan meetings and discharge planning.
• Maintains compliance with regulations and facility policies.
