Qualifications

  • Graduate of an accredited Physical Therapy program.
  • Current Physical Therapist license in the State of Ohio.
  • Experience in skilled nursing, long-term care, inpatient rehab, or healthcare setting preferred.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively with residents, families, physicians, and interdisciplinary team members.
  • CPR certification preferred.

Responsibilities

  • Evaluate residents upon admission and as needed to determine PT needs and goals.
  • Develop individualized treatment plans based on physician orders and resident needs.
  • Provide skilled PT services to improve strength, mobility, balance, and independence.
  • Assist with gait training, transfers, positioning, and mobility devices.
  • Document evaluations, plans, progress notes, and discharges accurately.
  • Collaborate with physicians, nursing, therapy team, families, and staff on care plans.
  • Monitor response to treatment and modify programs as needed.
  • Educate residents, families, and staff on therapy programs and safety.
  • Participate in care plan meetings and discharge planning.
  • Maintain compliance with regulations and standards.
  • Maintain therapy equipment and a safe treatment environment.

Physical Therapist (PT)

Position Summary

The Physical Therapist is responsible for evaluating residents, developing individualized treatment plans, and providing physical therapy services in accordance with physician orders, facility policies, and professional standards of practice. The Physical Therapist works closely with the interdisciplinary team to maximize resident outcomes and promote safe, effective rehabilitation services.

Responsibilities Include
  • Evaluates residents upon admission and as needed to determine physical therapy needs and functional goals.
  • Develops individualized treatment plans based on physician orders, resident needs, and therapy assessments.
  • Provides skilled physical therapy treatments to improve strength, mobility, balance, endurance, and functional independence.
  • Assists residents with gait training, transfer training, positioning, mobility devices, and therapeutic exercise programs.
  • Documents evaluations, treatment plans, progress notes, and discharge summaries accurately and timely.
  • Collaborates with physicians, nursing staff, therapy team members, families, and interdisciplinary staff regarding resident care plans and progress.
  • Monitors residents’ response to treatment and modifies therapy programs as appropriate.
  • Educates residents, families, and staff regarding therapy programs, safety precautions, and mobility techniques.
  • Participates in care plan meetings, discharge planning, and interdisciplinary team discussions.
  • Maintains compliance with federal, state, and facility regulations, policies, and professional standards.
  • Assists in maintaining therapy equipment and ensuring a safe treatment environment.
  • Supports facility goals related to quality outcomes, customer satisfaction, and rehabilitation success.
Qualifications Include
  • Graduate of an accredited Physical Therapy program.
  • Current Physical Therapist license in the State of Ohio.
  • Prior experience in skilled nursing, long-term care, inpatient rehab, or healthcare setting preferred.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively with residents, families, physicians, and interdisciplinary team members.
  • Must possess the ability to positively interact with residents, families, staff, visitors, and the general public.
  • Knowledge of rehabilitation techniques, therapy equipment, and documentation standards.
  • Ability to prioritize tasks and manage multiple responsibilities effectively.
  • CPR certification preferred.
