Qualifications

• Graduate from an accredited PT program.
• Current state PT license.
• Knowledge of SNF care standards and practices.
• Strong written and verbal communication and documentation skills.

Responsibilities

• Provides direct patient care, evaluates functional needs and outcomes.
• Designs and implements a plan of care based on thorough assessment.
• Conducts therapy within defined standards of care.
• Directs the assistance of support staff to achieve patient goals.
• Communicates effectively with other professionals and staff to optimize outcomes.
• Adheres to facility policies, safety, and infection control standards.
• Documents evaluations, treatment goals, and plans; maintains records.
• Pursues professional growth through education, workshops, and in-service training.
