Job summary

Emerald Care Center Southwest in Oklahoma City, OK seeks a dedicated Physical Therapist to assess, plan, and implement personalized rehabilitation programs that improve mobility and quality of life for residents.

You will collaborate with physicians, nurses, and other therapists, document progress, and stay current with best practices in physical therapy to ensure safe, effective care within a skilled nursing setting.

Qualifications

• Doctoral or Master’s degree in Physical Therapy from an accredited program.
• Current state licensure as a Physical Therapist.
• Experience in a skilled nursing facility preferred but not required.

Responsibilities

• Assess and evaluate residents to determine functional needs and goals.
• Develop and implement personalized rehabilitation programs to improve mobility and strength.
• Maintain thorough documentation of assessments, plans, progress and discharge summaries.
• Collaborate with physicians, nurses, and other therapists to coordinate care.
• Ensure safety and infection-control practices during therapy sessions.
• Engage residents and families to educate on exercises and home programs.
