Job summary

Emerald Care Center Tulsa is seeking a dedicated Physical Therapist to assess and plan personalized rehabilitation programs for residents. You will evaluate mobility, design therapy plans, and collaborate with physicians, nurses, and therapists to optimize outcomes.

A Doctoral or Master's in Physical Therapy and active state licensure are required. Prior SNF experience is preferred, with a patient-centered approach encouraged.

Qualifications

• Doctoral or Master's degree in Physical Therapy from an accredited program.
• Current state licensure as a Physical Therapist.
• Experience in a skilled nursing facility or similar setting preferred but not required.
• Strong clinical assessment skills and effective communication with patients and families.

Responsibilities

• Assess and evaluate residents to determine physical needs and goals.
• Develop and implement individualized therapy plans and exercise programs.
• Document assessments, plans, progress, and discharge summaries accurately.
