Job summary

Human Touch Home Health is seeking a PRN Physical Therapist to serve patients in Warrenton, Culpeper, and surrounding communities. You will conduct home PT evaluations, develop individualized plans, and provide gait, balance, and mobility training, while building your caseload around your availability.

Qualifications

• Active Virginia Physical Therapist License is required.
• Graduate of an accredited Physical Therapy program (DPT preferred).
• Valid Driver's License and reliable transportation.
• Strong clinical assessment and communication skills.
• Ability to work independently in patients' homes.
• Home Health experience preferred but not required.
• WellSky experience is a plus.

Responsibilities

• Complete physical therapy evaluations in patients' homes.
• Develop individualized plans of care.
• Provide therapeutic exercise, gait, balance, transfer, and mobility training.
• Assist patients to improve strength and functional independence.
• Assess home safety and fall risks.
• Educate patients and caregivers.
• Monitor progress and adjust treatment plans as needed.
• Coordinate care with physicians, nurses, therapists, and team members.
• Document accurately using WellSky and participate in reassessments.

What You'll Do
