Physical Therapist (PT) – Home Health

Locations: Taunton, MA.

Part‑Time / Per Diem Opportunities Available.

Paid per visit – routine visit rates starting at $80 per visit.

Key Responsibilities

• Evaluate patients’ physical conditions, limitations, and rehabilitation needs.
• Develop and implement individualized treatment plans.
• Provide therapy in accordance with the physician’s Plan of Care.
• Supervise and coordinate care with PTAs and HHAs.
• Maintain accurate documentation and progress notes.
• Educate patients and caregivers on exercises and equipment use.
• Assess home environments and recommend modifications.
• Collaborate with nurses, social workers, and other healthcare providers.
• Submit visit notes on time while maintaining HIPAA compliance.

Qualifications

• Licensed Physical Therapist (PT).
• Minimum 1 year of PT experience preferred.
• OASIS and WellSky (Kinser) experience preferred (training provided).
• Current CPR certification.
• Physical exam within the last 12 months.
• TB test within the last 12 months.
• Valid driver’s license.
• Must attend orientation at our Taunton, MA office.
