Job summary

Concept Rehab, a therapist-owned rehabilitation company with over 40 years of experience, is seeking a PRN Physical Therapist (PT) to join our Lima, Ohio team. This role offers flexible scheduling and the chance to work with an experienced rehab team.

You will evaluate and treat patients with mobility and functional challenges, develop individualized plans, and collaborate with an interdisciplinary team to deliver high-quality outcomes.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in Ohio.
• Experience in skilled nursing, post-acute care, rehabilitation, or geriatric settings preferred but not required.
• Current CPR certification preferred.

Responsibilities

• Evaluate patients for physical therapy services.
• Develop and implement individualized treatment plans.
• Provide skilled therapy interventions based on established plans of care.
• Supervise Physical Therapist Assistants in accordance with state practice regulations.
• Communicate effectively with physicians, nursing staff, families, and interdisciplinary team members.
• Participate in care planning and discharge planning activities.
• Document evaluations, progress notes, treatment notes, and discharge summaries accurately and timely.
• Educate patients, families, and facility staff on restorative and maintenance programs.
• Conduct in-service education and training for facility personnel.
• Maintain productivity and clinical quality standards.
