Job Opportunity: Physical Therapist (PT) - Per Diem

BAYADA Home Health Care is seeking an experienced Physical Therapist, PT, for a Per Diem role throughout Washington County, Maryland.

As a home care Physical Therapist, you will be a key member of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing to clients, enabling them to remain at home. Responsibilities:

1. Make home visits to clients within designated geographic areas.
2. Perform diagnostic tests and measurements, including mobility, joint range, transfer status, stability, ambulation patterns, muscle strength, balance, and safety assessments.
3. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and balance interventions.
4. Assess and update the care plan regularly and collaborate with nursing staff as part of the multidisciplinary team.
5. Educate clients, families, and caregivers on rehabilitative activities to promote health, safety, and independence.
6. Document observations, interventions, and evaluations accurately using electronic medical records via a touchpad tablet.

Qualifications:

1. Current Maryland Physical Therapist (PT) license.
2. Graduation from an American Physical Therapy Association-approved program.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Proficiency with computers; experience with EMR systems preferred.
