Job summary

Interim HealthCare of Central MT is seeking a licensed physical therapist to provide high-quality home-based care in the Helena, MT area. You will assess mobility, design and implement exercise programs, and work with a home health team to help patients regain function and independence.

This role offers a competitive hourly rate of $50-$65, travel reimbursement, uniform allowance, and flexible assignments designed to support work-life balance.

Qualifications

• Graduate of an accredited Physical Therapist Program.
• Active PT license in Montana.
• CPR certification is required.

Responsibilities

• Provide home-based physical therapy to patients who have mobility limitations.
• Collaborate with a home health team on patient plan of care and goals.
• Educate patients on exercises and self-care.
• Coach families on plan of care and safety during exercises.

As a Home Health Physical Therapist, here’s a big-picture view of what you’ll do:

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals
• Educate patients on their plan of care, established goals, exercises and self-care
• Coach family on the plan of care, therapy goals and safely assisting with exercises

A few must-haves for Home Health Physical Therapists:

• Graduate of an accredited Physical Therapist Program and active PT license in Montana
• Minimum of two (2) years of physical therapy experience, ideally in home healthcare
• CPR certification
• Hep B and TB immunizations
• Knowledge of state and federal home health regulations
• Good clinical judgement, strong interpersonal skills, dexterity and compassion
