Interim HealthCare of Central MT is seeking a Home Health Physical Therapist to deliver patient-centered PT in the home setting across Montana. You’ll collaborate with an integrated home health team to implement plans of care and optimize mobility and function for patients.

The role requires MT PT licensure, CPR certification, and at least two years of PT experience—preferably in home health—with flexible assignments and strong clinical judgment.

Qualifications

• Graduate of an accredited Physical Therapist Program and active MT license.
• Minimum of two years PT experience, ideally in home healthcare.
• CPR certification.
• Hep B and TB immunizations.
• Knowledge of state and federal home health regulations.
• Good clinical judgement, strong interpersonal skills, dexterity and compassion.

Responsabilités

• Provide home-based physical therapy to patients with mobility, strength or functional needs.
• Work with a home health team including RN, LPN, CNA, HHA, OT and SLP per plan of care.
• Educate patients on their plan, goals, exercises and self-care.
• Coach family on the plan of care and safe assistance with exercises.

As a Home Health Physical Therapist, here’s a big-picture view of what you’ll do:

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals
• Educate patients on their plan of care, established goals, exercises and self-care
• Coach family on the plan of care, therapy goals and safely assisting with exercises

A few must-haves for Home Health Physical Therapists:

• Graduate of an accredited Physical Therapist Program and active PT license in Montana
• Minimum of two (2) years of physical therapy experience, ideally in home healthcare
• CPR certification
• Hep B and TB immunizations
• Knowledge of state and federal home health regulations
• Good clinical judgement, strong interpersonal skills, dexterity and compassion
