Job summary

Senior Care Solutions is seeking a compassionate Physical Therapist (PT) for home health in the Melbourne area. You will deliver skilled PT services in patients’ homes, develop plans of care, and monitor progress to improve functional independence and quality of life, with an emphasis on safe, effective home-based interventions.

Ideal candidates hold a Florida PT license, CPR, and a PT degree from an accredited program.

Qualifications

• Current Florida PT license in good standing.
• Graduate of an accredited Physical Therapy program.
• Current CPR certification.
• Home health experience preferred, but not required.
• Strong clinical assessment and documentation skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and travel within the assigned service area.
• Comfortable using an EMR system for clinical documentation.

Responsibilities

• Provide skilled physical therapy evaluations and treatments to patients in their homes.
• Develop individualized treatment plans based on patient needs and physician orders.
• Perform assessments and establish appropriate goals for each patient.
• Provide therapeutic exercises, mobility training, gait training, balance activities, and other skilled interventions as appropriate.
• Educate patients and caregivers regarding treatment plans, home exercise programs, safety, and disease management.
• Monitor patient progress and modify treatment plans as needed.
• Communicate changes in patient condition and progress with the physician and interdisciplinary care team.
• Complete all required clinical documentation accurately and within required timeframes.
• Coordinate care with RNs, OTs, STs, MSWs, and other members of the interdisciplinary team.
• Participate in case conferences and care coordination as needed.
• Maintain compliance with Medicare, state, and agency requirements.
• Provide exceptional patient care while maintaining professional standards.

Position Overview

The Physical Therapist will provide skilled physical therapy services to home health patients based on their individual needs and established plans of care.
