Job summary

Choice Health at Home is seeking a PRN Physical Therapist in Tucson, AZ. You will assess, plan, organize, and provide rehabilitative services to improve mobility, relieve pain, and restore function in home health settings.

Responsibilities include initial evaluations, developing treatment plans, documenting progress, and coordinating with health professionals. A valid AZ PT license, CPR, and a flexible PRN schedule are required. Travel to patient homes will be routine.

Qualifications

• Successful completion of an accredited physical therapy program.
• Current, valid Physical Therapist license in the state of practice.
• Minimum of one year of home health experience preferred.
• Current CPR certification.
• Valid driver’s license and automobile insurance.
• Homecare Homebase experience preferred.
• Ability to work a flexible PRN schedule based on patient-care needs.

• Ensure continuity and quality of care through accurate, timely documentation.
• Communicate effectively with patients, families, team members, physicians, and other healthcare professionals.
• Promote and uphold the agency’s philosophy, policies, and procedures.
• Complete all required documentation within company-established guidelines.
• Accept patient assignments based on availability and organizational needs.

PRN Physical Therapist

As a PRN Physical Therapist, you will:

Physical Demands
