Job summary

Choice Health at Home is seeking a PRN Physical Therapist to provide rehabilitative services in patients’ homes. The role includes evaluating mobility, planning individualized treatment, and delivering care that improves function and reduces pain.

You will document evaluations, adjust plans based on patient response, and maintain infection-control standards while collaborating with families and healthcare teams. A valid PT license, CPR, and a flexible PRN schedule are required.

Qualifications

• Successful completion of an accredited physical therapy program.
• Current, valid Physical Therapist license in the state of practice.
• Minimum of one year of home health experience preferred.
• Current CPR certification.
• Valid driver’s license and automobile insurance.
• Homecare Homebase experience preferred.
• Ability to work a flexible PRN schedule based on patient-care needs.

Responsibilities

• Complete all required documentation within company-established guidelines.
• Accept patient assignments based on availability and organizational needs.

PRN Physical Therapist
