Job summary

Choice Health at Home is seeking a PRN Physical Therapist to provide rehabilitative services in patients’ homes. The role includes evaluating mobility, planning individualized treatment, and delivering care that improves function and reduces pain.

Qualifications

  • Current, valid Physical Therapist license in the state of practice.
  • Minimum of one year of home health experience preferred.
  • Current CPR certification.
  • Valid driver’s license and automobile insurance.
  • Homecare Homebase experience preferred.
  • Ability to work a flexible PRN schedule based on patient-care needs.

Responsibilities

  • Ensure continuity and quality of care through accurate, timely documentation.
  • Communicate effectively with patients, families, team members, physicians, and other healthcare professionals.
  • Promote and uphold the agency’s philosophy, policies, and procedures.
  • Complete all required documentation within company-established guidelines.
  • Accept patient assignments based on availability and organizational needs.

PRN Physical Therapist

Assess, plan, organize, and provide rehabilitative services that improve mobility, relieve pain, increase strength, and prevent or correct disabling conditions resulting from illness or injury.

As a PRN Physical Therapist, you will:
