Choice Health at Home in Oklahoma is seeking a compassionate Physical Therapist to join our growing home health team. You will help patients regain mobility, reduce pain, restore independence, and improve quality of life — all from patients' homes.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current and active Physical Therapist license.
• Minimum 1 year of home health experience preferred.
• CPR Certification required.
• Valid driver’s license and reliable transportation.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform and document comprehensive initial evaluations and develop individualized treatment plans.
• Assess patient needs and identify functional limitations prior to intervention.
• Design and implement rehabilitative programs to improve mobility, strength, balance, and pain management.
• Evaluate patient progress and modify treatment plans to maximize outcomes.
• Educate patients and caregivers on exercises, mobility strategies, and safety techniques.
• Maintain accurate and timely clinical documentation.
• Collaborate with physicians, nurses, and interdisciplinary care teams.
• Follow infection control and universal precaution standards during patient care.
• Support agency philosophy, mission, and quality care standards.

What You’ll Do

Physical Requirements

• Ability to stand, walk, sit, bend, squat, kneel, and climb for extended periods
