As a Physical Therapist / PT, you will:

1. Put your physical therapy skills to work—evaluate a patient's condition, develop a treatment plan, and help them improve daily. Also, instruct nursing staff and families on follow-through programs to sustain progress.
2. Communicate patient progress or issues to supervisors and team members; assist with scheduling and daily charge posting.
3. Document patient care in accordance with RehabCare, regulatory, licensing, payer, and accreditation standards.
4. Instruct patient's family or nursing staff in follow-through programs.
5. Maintain equipment and work areas in a safe, clean condition.
6. Make presentations to support marketing efforts at team conferences and in-services.
7. Handle responsibilities in line with the Company's Code of Business Conduct, compliance agreements, professional standards, and applicable laws.

The minimum requirements for this role include:

• Degree from an accredited Physical Therapy program.
• Valid National Provider Identification (NPI) number.
• Current and unrestricted Physical Therapy license in the state of practice.
• Current CPR certification.
• Strong organizational and communication skills.
• Ability to work flexible hours and weekends.
