Empath Home Health, a member of Empath Health, is seeking a PRN Home Health PT - Physical Therapist. The Physical Therapist treats patients using mechanical means such as exercise and heat to restore and maintain optimal physical functioning.

Locations include home health visits in Venice, Port Charlotte, Rotunda West, Englewood, North Port, Arcadia, Punta Gorda, and possibly north Sarasota county, based on patient needs.

The PRN Home Health PT will:

• Provide physical therapy services as prescribed by a physician, adhering to agency policies and regulations.
• Perform initial and ongoing assessments, including OASIS assessments, to evaluate patient functioning and daily living activities.
• Develop and update a care plan in collaboration with physicians and care team members to help patients achieve mobility goals.
• Report patient reactions and condition changes to physicians and agency representatives.

Requirements:

• Licensure as a physical therapist in Florida
• Preferred: at least one year of experience in home health care
