Qualifications

• Must hold an active Wisconsin Physical Therapist license.
• Experience with senior care in a skilled nursing setting is a plus.

Responsibilities

• Evaluate and document resident evaluations as per facility policies.
• Provide treatment to help residents regain or maintain function.
• Deliver direct patient care including therapeutic exercises.
• Educate patients and families on exercises and home programs.

Duties and Responsibilities: Physical Therapist (PT)

• Evaluate and document evaluations of referred residents in accordance with facility policies and procedures
• Provide appropriate treatment to help residents maintain or attain the highest level of function possible within the confines of his/her physical abilities
• Provide direct patient care, including therapeutic exercises and activities
• Educate patients and their families on therapeutic techniques and home exercises
• Collaborate with other healthcare professionals to ensure comprehensive patient care

Qualifications and Skills: Physical Therapist (PT)
