Job summary

Lifepoint Rehabilitation is seeking a Physical Therapist (PT) to join Ascension Saint Thomas Rehabilitation Hospital in Nashville, TN. This PRN role focuses on patient assessment, care planning, and delivering individualized physical therapy therapies.

You will supervise assistants, utilize standard PT modalities, and document patient progress. A Tennessee PT license and BLS within 30 days are required, with a commitment to high-quality patient care across Lifepoint Health.

Qualifications

• Current Physical Therapist license in Tennessee.
• Bachelor's degree in Physical Therapy required.
• Basic Life Support (BLS) certification to be obtained within 30 days of hire.

Responsibilities

• Perform initial and ongoing assessments of patient condition.
• Establish a care plan aligned with identified problems and patient goals.
• Plan and carry out individualized physical therapy programs.
• Administer therapy interventions using standard PT techniques and equipment.
• Direct and supervise PT assistants, students, and support staff.
• Assess competence, delegate tasks, and maintain clear communication channels.
• Apply physical agents and modalities (e.g., ultrasound, heat, light) as indicated.
• Evaluate and adjust prosthetic/orthotic devices when needed.

Ascension Saint Thomas Rehabilitation Hospital is a 40-bed inpatient acute care rehabilitation hospital located in Nashville, Tennessee.

How you'll contribute

A Physical Therapist (PT) who excels in this role:

• Performs initial and ongoing assessments of patient's condition.
• Establishes a plan of care which is appropriate to problems identified and involves the patient according to his or her capabilities and desires.
• Plans, prepares and carries out individually designed programs of physical treatment to maintain, improve or restore physical functioning, alleviate pain and prevent physical dysfunction in patients.
• Performs therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the age and condition of the patient.
• Directs and supervises physical therapy assistants, students, and other support personnel.
• Assesses competence, delegates specific tasks, and establishes channels of communication.
• Administers treatment involving application of physical agents, using equipment, moist packs, ultraviolet and infrared lamps, and ultrasound machines.
• Evaluates, fits, and adjusts prosthetic and orthotic devices and recommend modifications.
• Identifies and documents goals, anticipated progress and plans for reevaluation.

What we're looking for

Applicants should have a current Physical Therapist License in Tennessee in addition to:

• Education: Bachelor's Degree Required
• Licenses: Physical Therapist License in state of Tennessee
• Certifications: Basic Life Support (BLS) obtain within 30 days of hire
