Position Details

CommuniCare Rehab, part of the CommuniCare Family of Companies, has a PRN opening for a Physical Therapist (PT) at Allison Pointe Healthcare Center. Our therapists are In-House Team Members. This role offers the chance to make a direct impact on residents' lives through therapy. CommuniCare operates 140 centers across IN, MD, MO, OH, PA, VA, WV.

Qualifications

• Bachelor's, Master's, or Doctorate Degree in Physical Therapy from an accredited program

Job Responsibilities

• Evaluate, review, and screen patients' medical charts and evaluations under physician order; develop treatment plans tailored to patient needs
• Implement treatment plans through direct treatment and educate team members about the plan of care, providing supervision for PTAs when necessary
• Participate in interdisciplinary meetings, relay clinical findings, and make recommendations; conduct staff and family training as appropriate
