Qualifications

Responsibilities

• Evaluate residents to determine therapy needs and create individualized treatment plans.
• Provide direct physical therapy treatments in accordance with physician orders and resident goals.
• Document evaluations, progress notes, and treatment records accurately and timely.
• Collaborate with an interdisciplinary team to ensure comprehensive resident care.
• Supervise and support Physical Therapy Assistants (PTAs).
• Participate in discharge planning and caregiver education to ensure continuity of care.
• Maintain compliance with state, federal, and facility regulations.
