River Bend Nursing and Rehab in Evansville, IN is seeking a dedicated Physical Therapist (PT) to provide rehabilitation services to residents at our skilled nursing facility.

The PRN PT will evaluate residents, develop individualized treatment plans, deliver interventions to improve strength, mobility, balance, and functional independence, and collaborate with the nursing staff to support comprehensive resident care.

Qualifications

• Current Indiana PT license is required.
• Strong communication, organizational, and interpersonal skills.
• Ability to work effectively in an interdisciplinary rehabilitation team.

Responsibilities

• Provide quality physical therapy services and initiate referrals when appropriate.
• Evaluate residents and obtain data for treatment planning.
• Develop individualized treatment plans with target areas and goals.
• Provide PT interventions based on residents’ needs.
• Deliver therapeutic exercises to improve strength, mobility, balance, endurance, and independence.
• Instruct residents, families, and staff to carry over techniques into daily activities.
• Monitor progress and modify treatment approaches as needed.
• Maintain accurate documentation per clinical and regulatory requirements.
• Collaborate with nursing and interdisciplinary team to support resident care.

Location: River Bend Nursing and Rehab – Evansville, IN

River Bend Nursing and Rehab is seeking a dedicated Physical Therapist (PT) to provide rehabilitation services to residents at our skilled nursing facility in Evansville, IN.

The Physical Therapist is responsible for organizing and providing physical therapy services designed to facilitate rehabilitation, improve mobility and functional independence, and support residents’ overall quality of life.

Responsibilities

• Provide quality physical therapy services and initiate referrals when appropriate in accordance with established standards of practice, therapy department procedures, and productivity standards

• Evaluate residents and obtain the necessary data for treatment planning and implementation

• Develop individualized treatment plans that include target areas, short-term goals, and long-term goals

• Provide appropriate physical therapy interventions based on residents’ individual needs

• Provide therapeutic exercises and activities designed to improve strength, mobility, balance, endurance, and functional independence

• Instruct residents, families, and staff in appropriate physical therapy procedures to promote carryover of therapeutic techniques and goals into daily activities

• Monitor resident progress and modify treatment approaches as appropriate

• Maintain accurate and timely documentation in accordance with clinical and regulatory requirements

• Collaborate with nursing and other interdisciplinary team members to support comprehensive resident care

Qualifications

• Current license as a Physical Therapist (PT) in the State of Indiana

• Strong communication, organizational, and interpersonal skills
