Job summary

Sprenger Therapy is the in-house rehabilitation team for Sprenger Health Care Systems. We seek a Physical Therapist (PT) to deliver patient-centered care in our SNF setting and to develop individualized treatment plans with residents, families, and the interdisciplinary team.

You will provide services focused on restoring mobility, strength, function, and independence, with screenings, supervision of PTA staff, and timely referrals. New graduates welcome with mentorship available from day one.

Qualifications

• PTs must hold a degree in physical therapy from an accredited program.
• Current PT license or license-eligible.
• Commitment to patient-first, outcomes-driven care.
• Team-oriented mindset with strong communication skills.

Responsibilities

• Evaluate residents and develop individualized treatment plans with residents, families, and the interdisciplinary care team.
• Provide physical therapy services focused on restoring mobility, strength, function, and independence.
• Conduct screenings and identify residents who would benefit from intervention.
• Supervise and collaborate with PTA staff as applicable.
• Initiate referrals when appropriate and document clearly for quality outcomes.
