Job summary

Always Compassionate Health is seeking compassionate Physical Therapists to provide in-home rehabilitation in Nassau County, NY. This 1099 Independent Contractor role offers flexible per diem scheduling and immediate opportunities to help patients regain mobility and independence.

You will conduct evaluations, design personalized plans, guide caregivers, and monitor progress to ensure high-quality, patient-centered care in patients’ homes.

Qualifications

• Active New York State Physical Therapist license.
• Degree from an accredited Physical Therapy program.
• Strong clinical assessment and rehabilitation skills.
• Excellent communication and organizational skills.
• Previous physical therapy experience preferred.
• Ability to work independently and collaboratively.
• Commitment to compassionate, patient-centered care.

Responsibilities

• Conduct physical therapy evaluations to assess mobility, strength, balance, and functional limitations.
• Develop and implement individualized treatment plans based on patients’ needs and goals.
• Provide therapeutic exercises, gait training, balance training, and mobility interventions.
• Monitor patient progress and adjust treatment plans as needed.
• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility.
• Maintain accurate and timely clinical documentation.
• Collaborate with physicians, caregivers, and other healthcare professionals to coordinate patient care.
