Job summary

Nassau Rehabilitation and Nursing Center in Hempstead, NY is seeking a dedicated Physical Therapist to join our interdisciplinary rehabilitation team in a full-time capacity. This position offers meaningful patient impact in a supportive skilled nursing environment.

You will evaluate patients, design and implement individualized treatment plans, guide therapeutic exercises, and educate families on at-home recovery.

Qualifications

• DPT from an accredited program.
• Active state licensure as a Physical Therapist.
• 2+ years of experience preferred in nursing home settings.
• Experience with SigmaCare or Net Health is a plus.

Salary: $50–$60 per hour (based on experience)

Nassau Rehabilitation and Nursing Center is seeking a dedicated and compassionate Physical Therapist to join our interdisciplinary rehabilitation team. This full-time role offers the opportunity to make a meaningful impact on patient recovery and quality of life in a supportive skilled nursing environment.

Key Responsibilities

• Consult with patients to evaluate physical conditions, symptoms, and medical history

• Diagnose movement dysfunction through observation and assessment

• Develop and implement individualized treatment plans

• Coach patients in therapeutic exercises and stretches

• Provide hands‑on therapy (e.g., massage) to relieve pain and promote healing

• Monitor progress, adjust treatment plans, and maintain accurate patient records

• Educate patients and families on in‑home care and recovery expectations

Candidate Requirements

• Doctor of Physical Therapy (DPT) from an accredited program

• Active state licensure as a Physical Therapist

• Minimum of 2 years of experience preferred

• Nursing home or skilled nursing experience preferred

Critical Qualifications

• Proficiency in physical therapy techniques and modalities

• Strong assessment and diagnostic skills

• Compassion, patience, and excellent communication abilities

• Familiarity with SigmaCare and Net Health preferred
