Nassau Rehabilitation and Nursing Center is seeking a dedicated Physical Therapist (PT) to join our interdisciplinary rehabilitation team in Hempstead, NY. This full-time position offers the opportunity to positively impact patient recovery and quality of life in a skilled nursing setting.

The ideal candidate holds a DPT, active licensure, and prefers 2+ years in a nursing home environment. Proficiency in PT techniques and familiarity with SigmaCare/Net Health are desirable.

Qualifications

• Doctor of Physical Therapy (DPT) from an accredited program.
• Active state licensure as a Physical Therapist.
• Minimum of 2 years of experience preferred.
• Nursing home or skilled nursing experience preferred.
• Proficiency in physical therapy techniques and modalities.
• Strong assessment and diagnostic skills.
• Compassion, patience, and excellent communication abilities.
• Familiarity with SigmaCare and Net Health preferred.

Responsibilities

• Consult with patients to evaluate physical conditions, symptoms, and medical history.
• Diagnose movement dysfunction through observation and assessment.
• Develop and implement individualized treatment plans.
• Coach patients in therapeutic exercises and stretches.
• Provide hands-on therapy (e.g., massage) to relieve pain and promote healing.
• Monitor progress, adjust treatment plans, and maintain accurate patient records.
• Educate patients and families on in-home care and recovery expectations.
