Centra Healthcare Solutions Inc. is seeking an experienced or newly graduated Physical Therapist to join a growing SNF/LTC team on a contract assignment in Saluda, South Carolina.

This is a contract position with days schedule. The role requires developing a plan of care for each patient, providing skilled PT services per physician orders, assessing needs, adjusting care to meet goals, and collaborating with all disciplines to plan and evaluate team objectives.

Qualifications

• Graduate from an accredited PT program.
• Current state PT license or in the process of licensure.
• Develop and implement plan of care per physician orders.

Requirements include:

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
