Job summary

Centra Healthcare Solutions Inc is seeking an experienced or newly graduated Physical Therapist (PT) for a contract assignment in Colorado. The role involves days schedule and requires licensure as a PT or active licensure in process, plus CPR certification.

The PT will develop plans of care, provide skilled services as ordered, assess patient needs, and collaborate with the care team to meet goals. The position emphasizes accountability for aligning with the organization’s vision and goals

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or in the process of licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
