Job summary

Centra Healthcare Solutions Inc is seeking an experienced or newly graduated Physical Therapist for a contract assignment at a SNF/LTC.

The PT will develop plans of care, provide skilled services per physician orders, assess patient needs, and collaborate with the care team. CPR certification and an active PT license are required; this is a day-shift, on-site contract with competitive pay.

Qualifications

• Must have graduated from an accredited program.
• Current state license as a Registered Physical Therapist (PT) or actively in process for licensure.
• Current CPR certification.

Requirements include:

• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
