Job summary

Centra Healthcare Solutions Inc seeks an experienced or newly graduated Physical Therapist (PT) for a contract assignment at a skilled nursing facility/long-term care in Modesto, CA. The PT will join a growing team and provide PT services in accordance with physician orders, developing care plans and collaborating with the multidisciplinary team.

Requirements include graduation from an accredited school and a current state PT license (or in process).

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or in process for licensure.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions per physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals.
• Collaborate with all disciplines to plan and evaluate patient goals.

A great Skilled Nursing Facility (SNF) / Long Term Care (LTC) is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contract assignment!This is a contract position.Days schedule.

This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
