Job summary

Centra Healthcare is seeking a licensed Physical Therapist to join a SNF/LTC setting in Globe, AZ on a contract basis. You will evaluate patients, develop plans of care, and provide skilled PT services per physician orders.

The position requires graduation from an accredited program, current PT license, CPR certification, and the ability to collaborate with the care team while delivering compassionate, standards-driven care. Day schedules available.

Qualifications

• Graduation from an accredited physical therapy program.
• Current state license as a Physical Therapist (PT) or actively pursuing licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services in accordance with physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals of PT interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.

Job Description:

A great SNF / LTC is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contract assignment!

• This is a contract position.
• Days schedule.
• This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assess patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
