Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or actively pursuing licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions per physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals.
• Collaborates with all disciplines to plan and evaluate team goals.

Responsibilities

• Develop and implement PT treatment plans based on orders and assessments.
• Deliver skilled PT interventions and document patient progress.
• Collaborate with nursing and therapy teams to coordinate care.
• Monitor patient response and adjust plans as needed.
