Job summary

Centra Healthcare Solutions Inc is seeking a Physical Therapist (PT) for a contract assignment with a day schedule. The role requires recent graduation or experience and a valid PT license, plus CPR certification.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or actively pursuing licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions per physician orders.
• Assess patient needs, plan, evaluate and modify care for PT goals.
• Collaborate with disciplines to plan and evaluate team goals.

Responsibilities

• Develop and implement individualized PT plans of care.
• Deliver therapeutic PT interventions in line with physician orders.
• Monitor patient progress and adjust plans accordingly.
• Collaborate with caregivers and other professionals to optimize patient outcomes.

Requirements
