Job summary

THHN Consulting LLC, a Florida-based healthcare consulting company, seeks licensed Physical Therapists to provide home health therapy across Florida as 1099 independent subcontractors. You will conduct evaluations, develop treatment plans, and deliver skilled PT services in the home health setting — all on a schedule you control.

You will perform initial assessments, create care plans, and document with OASIS while coordinating with the care team and ensuring Medicare/Medicaid compliance.

Qualifications

• Active, unrestricted Physical Therapy license in Florida.
• Current CPR/BLS certification (BLS for Healthcare Providers).
• Proof of professional liability insurance.
• Valid W-9 (blank provided if needed).
• Reliable transportation to patient homes.
• Strong documentation skills; home health or OASIS experience preferred.

Responsibilities

• Perform physical therapy evaluations, start-of-care assessments, and OASIS documentation.
• Develop and implement individualized patient care plans.
• Provide therapeutic exercise, manual therapy, and functional mobility training.
• Complete re-evaluations, recertifications, and discharge documentation.
• Coordinate with the interdisciplinary care team as needed.
• Maintain accurate, timely documentation in compliance with Medicare/Medicaid guidelines.
