THHN Consulting LLC is seeking licensed Physical Therapists to provide home health therapy to patients across Florida as 1099 independent subcontractors. You will conduct evaluations, develop treatment plans, and deliver skilled PT services in the home health setting — all on a schedule you control.

You will perform evaluations, complete OASIS documentation, and coordinate with the care team, with per-visit compensation and flexible scheduling.

Qualifikationen

• Active, unrestricted Florida Physical Therapy license.
• Current CPR/BLS certification.
• Proof of professional liability insurance.
• Valid W-9 on file; reliable transportation.
• Home health or OASIS experience preferred.
• Strong documentation skills; home health or OASIS experience preferred.

• Perform PT evaluations, start-of-care assessments, and OASIS documentation.
• Develop and implement individualized patient care plans.
• Provide therapeutic exercise, manual therapy, and functional mobility training.
• Complete re-evaluations, recertifications, and discharge documentation.
• Coordinate with the interdisciplinary care team and maintain Medicare/Medicaid documentation.
• Maintain accurate, timely documentation in compliance with Medicare/Medicaid guidelines.
