Job summary

Lifepoint Health, in partnership with UW Health Rehabilitation Hospital, seeks a full-time Physical Therapist to join our Madison, Wisconsin team. You will assess, treat, and educate patients to restore strength, mobility and function, with discharge planning and collaboration with rehab colleagues.

Qualifications include graduation from an accredited PT program, current PT license, CPR certification, and prior rehab experience preferred.

• Experience in rehab setting preferred.
• Current PT license.
• CPR certification required.

Responsibilities

• Conducts physical therapy assessments and develops treatment plans.
• Provides direct patient care to restore strength, mobility, and function.
• Educates patients and caregivers on home exercise programs.
• Documents treatment sessions and patient outcomes accurately.
• Collaborates with rehab teams and participates in discharge planning.
• Performs other duties as assigned.
