Job Opportunity: Physical Therapist (PT) at BAYADA Home Health Care

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time role providing home health visits at our senior living office in Blue Bell, PA. This office serves adult and geriatric clients across territories in the Northeast Philadelphia/Eastern Montgomery County area.

As a home care Physical Therapist, you will be a key member of a multidisciplinary healthcare team delivering rehabilitative care and skilled nursing, enabling clients to remain at home with appropriate medical support.

Responsibilities:

1. Make home visits to clients in designated areas.
2. Perform diagnostic tests such as mobility, joint range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
3. Implement wound care interventions using evidence-based dressings and topical agents.
4. Develop and update personalized care plans, including manual exercises, gait training, and balance interventions.
5. Assess and revise care plans regularly, collaborating with nursing staff as needed.
6. Educate clients and families on rehabilitative activities to promote health, safety, and independence.
7. Document all observations and interventions accurately using electronic tablets.

Qualifications:

• Valid PA Physical Therapist (PT) license.
• Graduation from an American Physical Therapy Association-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; prior EMR experience preferred.
