Qualifications

Job description

Empath Home Health, a member of Empath Health, is seeking a PRN Home Health PT (Physical Therapist). The Home Health PT treats patients using mechanical means such as exercise and heat to restore and maintain optimal physical functioning.

Locations

Home health visits are available in east Pasco County (Wesley Chapel, Land O' Lakes, Odessa, Dade City, Zephyrhills, etc.). Visits may also be available in west Pasco, Pinellas, or Hillsborough counties.

Schedule

PRN (as needed)

Responsibilities

• Provide physical therapy services to patients as prescribed by a physician, in compliance with agency policies, procedures, and regulations.
• Perform initial and ongoing assessments, including OASIS assessments, to determine patients' levels of functioning and ability to perform daily activities.
• Develop and revise care plans in consultation with physicians and care team members to help patients achieve mobility goals.
• Report patient reactions to treatment and any changes in condition to the physician and agency representatives.

Requirements

• Licensure as a physical therapist in Florida
• Preferred: Minimum of one year of experience in home health care
