Interim HealthCare Inc. is seeking a Physical Therapist (PT) for home health in Summit and Medina Counties, OH. This full-time role offers a $90,000 annual salary and the opportunity to provide one-on-one patient care in patients' homes.

You will evaluate and treat patients, develop individualized treatment plans, monitor progress, educate families, and coordinate with a healthcare team to ensure optimal outcomes. Strong communication and clinical skills are essential.

Qualifications

• Licensure: Current, unrestricted license to practice as a Physical Therapist (PT) in the state associated with this position.
• Certification: Current CPR/BLS certification.
• Experience: One (1) year of professional experience practicing as a Physical Therapist in a home health or similar clinical setting.
• Reliable transportation to and from patient homes throughout Summit and Medina Counties.
• Strong clinical assessment, communication, and patient care skills.

Responsibilities

• Evaluate and treat patients experiencing physical disabilities or limited mobility due to injury, illness, disease, or surgery in a home health setting.
• Develop and implement individualized treatment plans using therapeutic exercises, stretching, manual therapy, and specialized equipment to reduce pain and improve mobility.
• Assess patients' progress and adjust treatment plans as needed to achieve optimal outcomes.
• Educate patients and family members on rehabilitation techniques, exercises, and activities that promote safety, health, and independent living.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate care and achieve the best possible patient outcomes.
• Complete accurate and timely documentation of evaluations, treatments, and patient progress.
