Interim HealthCare Inc. in New Philadelphia, OH seeks a full-time Physical Therapist to provide home health care across New Philadelphia and nearby counties.

You will evaluate and treat patients in their homes, establish individualized plans, and educate families to promote independence and safety. This role emphasizes one-on-one patient care, collaboration with other providers, and about 30 patient visits per week, with coverage in Tuscarawas, Coshocton, and Holmes Counties.

Qualifications

• Current, unrestricted license to practice as a Physical Therapist (PT) in the state associated with this position.
• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation to and from patient care locations.
• One year of professional experience practicing as a Physical Therapist in a home health or similar setting.

Responsibilities

• Evaluate and treat patients experiencing physical disabilities or limited mobility due to injury, disease, or surgery in a home care environment.
• Establish and administer individualized treatment plans using exercises, stretching, manual therapy, and equipment to manage pain and improve mobility.
• Educate patients and family members on rehabilitative care and activities that promote health, safety, and independent living.
• Collaborate with other providers involved in each patient's care to achieve the best possible outcomes.
• Provide approximately 30 patient visits per week to support a full-time schedule.
• Assist with vacation coverage in the Tuscarawas, Coshocton, and Holmes County areas as needed.
