Interim HealthCare Inc. is seeking a dedicated Physical Therapist to provide in-home care in the Altoona area. You will evaluate and treat patients, create and implement a personalized plan, and help improve mobility and independence.

The role is PRN per-diem with flexible scheduling, 1:1 patient care, and competitive pay. A state PT license, CPR/BLS/First Aid, and reliable transportation are required to support patients safely in their homes.

Qualifications

• Current unrestricted PT license in the state of posting.
• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation to care sites.
• At least 1 year of PT experience in home health or similar setting.

Responsabilités

• Evaluate and treat patients in home care to improve mobility.
• Establish and administer a treatment plan using exercises and manual therapy.
• Educate patients and families in rehabilitative care for health and safety.
• Collaborate with other providers to achieve optimal outcomes.

As a Physical Therapist (PT), you will:

• Evaluate and treat patients suffering from physical disability or lack of mobility due to injury, disease, or surgery in a home care environment
• Establish and administer a treatment plan using exercises, stretching, manual therapy, and equipment to manage patients' pain and increase mobility
• Educates patients and family members in rehabilitative care and activities necessary to promote health, safety, and independent living
• Collaborate with other providers servicing your client(s) to reach the best possible patient outcomes

To qualify as a Physical Therapist (PT) with us, you will need:

• Licensure: Current unrestricted license to practice as a Physical Therapist (PT) in the state associated with this position.
• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation to/from care sites and/or work locations
• One (1) year of professional experience practicing as a Physical Therapist (PT) in a home health or similar setting.
