Job summary

Mission Healthcare seeks a licensed Physical Therapist to provide home health services in the Gresham, OR area. You will evaluate patients, design personalized therapy plans, and work with an interdisciplinary team to improve mobility and reduce pain.

Requirements include an active PT license, a Master's or Doctorate in Physical Therapy, and at least 1 year of experience (home health preferred); CPR/BLS and a valid driver's license are essential.

Qualifications

• Active Physical Therapist license in practicing state.
• Master's or Doctorate degree in Physical Therapy.
• Minimum of 1 year experience in physical therapy; home health preferred.
• Current BLS or hands-on CPR certification.
• Valid driver's license and reliable vehicle.

Responsibilities

• Evaluate and assess patients' physical conditions; develop individualized treatment plans.
• Implement therapeutic exercises to relieve pain and improve mobility.
• Educate patients and caregivers on exercises, safety, and self-care.
• Collaborate with interdisciplinary team to provide holistic care.
• Maintain accurate documentation of patient progress and plans.

Pay range: Starting at $65/hour, depending on experience. $5K Sign On!

Schedule/Shift: Monday-Friday or 4 day work week!

Territory/Location: Gresham, OR and surrounding.

• Active Physical Therapist licensein practicing state.
• Master's or Doctorate degreein Physical Therapy.
• Minimum of 1 year of experienceas a Physical Therapist, home health experience strongly preferred.
• Current BLS, hands‑on CPR certification.
• Valid driver's license and reliable, insured vehicle.
