Job summary

Mission Healthcare is seeking a licensed Physical Therapist for Home Health in Gresham, OR. You will evaluate patients, create individualized treatment plans, and implement therapeutic exercises to enhance mobility and reduce pain.

The role offers a 4-day work week option or traditional Mon-Fri schedule, with a $5K sign-on and a starting pay of $65/hour. Responsibilities include educating patients and caregivers, collaborating with an interdisciplinary team, and thorough progress documentation.

Qualifications

• Licensed Physical Therapist in practicing state.
• 1 year of physical therapy experience preferred, home health a plus.
• Active BLS/CPR certification.

Responsibilities

• Evaluate and assess patients' physical conditions and develop individualized treatment plans.
• Implement therapeutic exercises and techniques to relieve pain and improve mobility, strength, and coordination.
• Educate patients and caregivers on exercises, safety precautions, and self-care strategies.
• Collaborate with an interdisciplinary team to provide holistic, patient-centered care.
• Maintain accurate and timely documentation of patient progress and treatment plans.

• Valid driver's license and reliable, insured vehicle.
